IAS 2017 - MK-8591: efficacia antivirale e tollerabilità in un piccolo studio in aperto

Merck ha presentato a Parigi i risultati di uno studio proof of concept di fase Ib: Single doses as low as 0.5 mg of the novel NRTTI MK -8591 suppress HIV for at least seven days.

30 persone con HIV e naive hanno ricevuto ciascuna una singola dose, ma nel complesso dosaggi diversi (30 mg, 10 mg, 2 mg, 1 mg, 0.5 mg) di MK-8591 e nel corso dei successivi 7 giorni per i 3 dosaggi più bassi e 10 giorni per i 2 più alti è stato valutato l'andamento delle viremie con l'obiettivo di stabilire il dosaggio più basso in cui si ottenga un declino medio della viremia di almeno 1,5 log. Si sono inoltre indagati la farmacocinetica e i marker di sicurezza.
Dopo i 7 e 10 giorni, a tutti i partecipanti si è suggerito di iniziare un regime antiretrovirale standard.

A tutti i dosaggi il farmaco ha causato una rapida e robusta diminuzione delle viremie: 168 ore dopo la somministrazione, chi aveva ricevuto il dosaggio più basso di 0,5 mg ha avuto una diminuzione media della viremia di 1,18 log. Al dosaggio più alto, quello di 30 mg, si è visto un declino continuo della viremia per tutti i 10 giorni e la diminuzione media è stata di 1,57 log.
Dove è stato possibile valutare le mutazioni del virus, non si sono trovate le mutazioni comuni che inducono resistenza agli NRTI (compresa la M184V/I, responsabile di tanti fallimenti terapeutici).

La farmacocinetica è stata analoga a quella che si è vista nei volontari sani di uno studio precedente: l'emivita del farmaco nelle PBMC è stata di 78,5 - 128 ore, e questo lo rende compatibile con una varietà di possibili regimi terapeutici.

Tutti i dosaggi in genere sono stati ben tollerati, con pochi eventi avversi di grado leggero/moderato (i più comuni: mal di testa, sintomi da raffreddore, diarrea e vomito). Non si sono potute stabilire correlazioni fra la dose di farmaco assunta e la frequenza o la severità degli eventi avversi.
Non si sono viste alterazioni nei parametri vitali, né anomalie di laboratorio.

È stato inoltre portato al congresso un abstract - Weekly oral MK-8591 protects male rhesus macaques against repeated low dose intrarectal challenge with SHIVC109P3 - in cui si dimostra che l'MK-8591 ha offerto una protezione completa a 8 macachi a fronte di ripetuti tentativi di infettarli per via rettale con basse dosi di un virus chimera SHIV. Gli animali di controllo, invece, si sono infettati tutti.
Questo apre la strada a una sperimentazione del farmaco come PrEP.

Più dosi al giorno di MK-8591 somministrati ad adulti sani hanno dimostrato
1) di essere generalmente ben tollerati (fino a 5 mg/giorno per 6 settimane)
2) di raggiungere nelle PBMC, nel retto e nella vagina livelli considerati efficaci per attività antivirale.
Ne segue che si ritiene possa esercitare un'attività antivirale a dosaggi giornalieri anche di soli 0,25 mg.

Il farmaco è attualmente studiato in combinazione con doravirina o lamivudina in un trial multicentrico di fase IIb, randomizzato, in doppio cieco e con placebo: MK-8591 With Doravirine and Lamivudine in Participants Infected With Human Immunodeficiency Virus Type 1 (MK-8591-011) (DRIVE2Simplify).
Il trial, iniziato a fine 2017, darà i primi risultati fra un anno.

Un nuovo farmaco (unico nel suo genere mi sembra di capire) è sempre una bella notizia! Pensavo lo studiassero per una modalità long acting (però magari avrebbero dovuto fare una partnership con la janssen o la viiv per cui hanno optato per iniziare con una biterapia in-house...)

Sì, ma per adesso come PrEP da somministrarsi una volta a settimana. E siamo ancora allo studio su scimmie:

  • Background: MK-8591 (4’-ethynyl-2-fluoro-deoxyadenosine; EFdA), a nucleoside reverse transcriptase translocation inhibitor (NRTTI), was previously shown to completely protect rhesus macaques (RM) against SHIV infection following intrarectal (IR) challenge when dosed at 3.9 mg/kg weekly. At this dose, the mean intracellular MK-8591-triphosphate (TP) concentration of 805 fmol/10Λ6 PBMCs at time of challenge exceeds the lowest MK-8591-TP C168hr that demonstrated potent antiviral activity in HIV infected patients. To determine the lowest drug levels that confer protection, RMs were challenged at progressively lower doses until protection was no longer observed.
    Methods: The eight male RM that had been successfully treated with 3.9 mg/kg MK-8591 after 20 weeks (w) were dosed with 1.3 mg/kg MK-8591 orally on day 0 and qw for 6 doses. All animals were again challenged IR with 1 mL of 50TCID50 of SHIVC109P3 on day 6 and weekly thereafter for up to 4 challenges or until infection was confirmed. Prior to challenge, blood was drawn for virology and PK. Infection was confirmed by real-time RT PCR in plasma. Proviral DNA was measured qw by PCR and virus-specific antibody responses were assessed. Intracellular levels of MK-8591- TP were measured by LC/MS/MS. After a 4 w washout, the treatment sequence was repeated on uninfected RM, (0.43 mg/kg weekly for 6 doses) with challenge on day 6 and weekly for up to 4 challenges or until infection was confirmed. After 10 w, this was repeated on the remaining uninfected RM at a dose of 0.1 mg/kg.
    Results: All 8 animals remained uninfected after challenges at the 1.3 & 0.43 mg/kg dosing levels. At 0.1 mg/kg, 2/8 animals became infected, with one animal infected after 3 challenges and the other after 4 challenges. Mean levels of intracellular MK-8591-TP at the time of challenge were 282 and 102 fmol/10Λ6 PBMCs at the 1.3 and 0.43 mg/kg dosing levels, respectively. At the 0.1 mg/kg dose MK-8591-TP levels are estimated to be ~24 fmol/10Λ6 PBMCs.
    Conclusion: In rhesus macaques MK-8591 completely protects against SHIV infection at weekly doses of 1.3 and 0.43 mg/kg and is partially protective at 0.1 mg/kg (HR 7.2, p=0.0004). MK-8591-TP levels that are protective in this model are achievable in humans at weekly doses of less than 250 μg weekly or 10 μmg daily, suggesting MK-8591 utility in extended duration prophylaxis against HIV infection.

IAS 2019 - l'inibitore nucleosidico della traslocazione della trascrittasi inversa (NNRTI) MK-8591 (ora si chiama Islatravir) in combinazione con doravirina


Merck ha presentato a IAS 2019 i risultati del trial di fase IIb su efficacia, tollerabilità e sicurezza di islatravir (MK-8591) in combinazione con il suo inibitore non-nucleosidico della trascrittasi inversa doravirina e in confronto con la combinazione fissa doravirina 100 mg/lamivudina (3TC) 300 mg/tenofovir disoproxil fumarato 300 mg (DELSTRIGO™). È il primo famaco il cui nome si costruisce sulla radice "travir", che indica gli inibitori nucleosidici della traslocazione della trascrittasi inversa.

Diversi dosaggi del farmaco sperimentale sono stati testati in adulti con infezione da HIV mai prima trattata con antiretrovirali e, ad ogni dosaggio, la combinazione ISL + DOR ha mantenuto a 48 settimane un'efficacia antivirale paragonabile a quella della combinazione a tre farmaci DELSTRIGO (misurata dal numero di partecipanti che raggiungevana HIV RNA <50 copie/mL).

Criteri di inclusione nel trial: CD4 >200 cellule/mm3 and HIV RNA ≥1,000 copie/mL, naive.
Caratteristiche dei partecipanti: età media 31 anni, 93% uomini, 76% bianchi, 20% neri, CD4 medi 492, 22% aveva VL >100.000.
40% dei partecipanti trattati in nord America, 30% in Sud America, 25% in Europa.

Nello studio, 121 partecipanti sono stati assegnati in modo randomizzato (1:1:1:1) a 4 gruppi con diversi dosaggi del farmaco una volta al giorno. Dopo un minimo di 24 settimane, quelli dei gruppi che ricevevano islatravir e avevano raggiunto VL irrilevabile e non avevano avuto fallimenti virologici sono stati spostati in un gruppo di biterapia ISL + DOR (entrambi con dosaggi di 100 mg).

Le riduzioni medie delle viremie a 24 settimane sono state di 2,9 log nel gruppo ISL e di 2,8 log nel gruppo DELSTRIGO.

A 48 settimane, l'efficacia del regime a due farmaci DEL/DOR è stata paragonabile a quella del regime a tre farmaci DOR/3TC/TDF. I dati parziali dell'estesione del trial a 96 settimane sembrano confermare la buona efficacia virologica.

La tollerabilità è stata buona: a 48 settimane, non si sono visti eventi avversi gravi attribuibili al farmaco sperimentale (due eventi avversi seri, una paralisi facciale e una moderata dissenteria, non sono stati attribuibili a ISL).
Gli eventi avversi più comuni, riferiti da almeno il 10% dei partecipanti, sono stati:

  • - nel gruppo a 0.2 mg (29 pz), sinusite, dolori alle estremità, mal di testa
    - nel gruppo a 0.75 mg (30 pz), diarrea, nausea, bronchite, nasofaringite, sifilide, carenza di vitamina D
    - nel gruppo a 2.25 mg (31 pz), artralgia e mal di testa - in questo gruppo ci son o stati 2 abbandoni


Questi dati permettono a Merck di decidere di passare a trial di fase III.

Through 48 weeks of a phase 2 dose-ranging study, the novel antiretroviral islatravir had a good safety profile when combined with the nonnucleoside doravirine [1]. The two-drug regimen had modest effects on metabolic variables and body weight.

Islatravir inhibits HIV reverse transcription via two mechanisms—translocation inhibition (which stops the reverse transcriptase nucleotide binding site from opening) and delayed chain termination (which prevents nucleotide incorporation into viral DNA). This one-two punch against HIV may contribute to the high potency of islatravir against drug-resistant virus and its high barrier to resistance. A phase 2 study also presented at AIDS 2020 found a low rate of virologic failure with islatravir plus doravirine through 48 weeks [2]. Researchers analyzed islatravir/doravirine safety in the same phase 2 trial.

Study P011 enrolled antiretroviral-naive adults and randomized them to doravirine plus islatravir (at 0.25, 0.75, or 2.25 mg once daily) or to a control regimen of doravirine plus lamivudine and tenofovir disoproxil fumarate (3TC/TDF). Participants randomized to islatravir/doravirine also took 3TC for 24 weeks then dropped 3TC if they reached a viral load below 50 copies.

Each of the four study arms had about 30 participants. In the 0.25-, 0.75, and 2.25-mg islatravir groups, numbers of drug-related adverse events through week 48 were 0, 3, and 4, compared with 6 in the control arm (7.8% for combined islatravir arms versus 19.4% in the control arm). Numbers of 48-week serious adverse events in the 0.25-, 0.75, and 2.25-mg islatravir groups were 1, 2, and 0, compared with 2 in the control arm (3.3% combined islatravir arms versus 6.5% control arm). Two people taking 2.25 mg of islatravir plus doravirine stopped treatment because of an adverse event, as did 1 in the control arm (2.2% combined islatravir arms versus 3.2% control arm).

The most frequent adverse events across all four treatment groups were headache (11.1% in combined islatravir arms, 6.5% in control arm), nausea (8.9% combined islatravir arms, 9.7% control arm), and diarrhea (6.7% combined islatravir arms, 16.1% control arm). Most cases of diarrhea were mild, transient, and judged not related to study treatment. The same can be said for headache. Arthralgia occurred in 1 person taking 0.25-mg islatravir, 2 taking 0.75-mg islatravir, 4 taking 2.25-mg islatravir, and 1 taking the control regimen (7.8% combined islatravir arms, 3.2% control arm).

The most frequent lab abnormality through 48 weeks was elevated creatine kinase (grade 4, 5.6% in combined islatravir arms, 3.2% in control arm), which was linked to physical exertion in 9 of 10 cases, all of which resolved. Over 48 weeks rates of grade 3 triglyceride elevations were 2.6% in the combined islatravir arms and 0 in the control arm. Respective rates of 48-week grade 3 abnormal alanine aminotransferase were 3.3% and 3.2%. Through 48 weeks total-to-HDL cholesterol ratio measured -0.26 in the combined islatravir arms and -0.31 in the control arm. Glucose rose an average 2.3 mg/dL in the combined islatravir arms while falling 2.0 mg/dL in the control arm.

Through 48 weeks, 34 of 90 people (37.8%) in the combined islatravir arms and 7 of 31 (22.6%) in the control arm had more than a 5% weight gain. Most of the weight gain came in the first 24 weeks and so, the researchers suggested, may reflect the return-to-health effect of initial antiretroviral therapy.

The researchers concluded that “islatravir was well tolerated regardless of dose through 48 weeks of treatment, with fewer drug-related adverse events than doravirine/3TC/TDF.” Ongoing phase 2 and 3 trials of islatravir/doravirine use the 0.75-mg dose of islatravir.

Gli ultimi dati presentati al vCROI 2021 sul lenacapavir in persone con HIV multiresistente e già tanti regimi di cART alle spalle sono molto incoraggianti, perché il tasso di soppressione delle viremie a 26 settimane dall'unica iniezione dell'inibitore del capside unita a un regime ottimizzato di antiretrovirali si è mantenuto alto:
il 73% dei partecipanti allo studio CAPELLA (n=19/26) ha ottenuto una carica virale non rilevabile (<50 copie/mL)
Oggi Gilead e Merck hanno rilasciato un comunicato stampa congiunto molto interessante, in cui annunciano di aver deciso di sviluppare e commercializzare insieme un regime long-acting combinato, che vede Gilead mettere il suo lenacapavir e Merck il suo islatravir, inibitore nucleosidico della traslocazione della trascrittasi inversa (NRTTI).
I primi trial sulla combinazione orale dei due farmaci inizieranno nella seconda metà di quest'anno.

Comunicato stampa di Merck per annunciare che due trial di fase III (ILLUMINATE - SWITCH A e B) di islatravir + doravirina (Pifeltro) sono andati bene e hanno soddisfatto gli obiettivi prefissati: la pillola dei due farmaci combinati, da assumere una volta al giorno, ha dimostrato a 48 settimane efficacia antivirale comparabile a quella dei due regimi classici con cui è stata messa a confronto.
Quanto si è visto su sicurezza e tollerabilità conferma il buon profilo già visto nei trial di fase II.

Gli studi ILLUMINATE sono sperimentazioni complesse, che includono un gran numero di partecipanti in situazioni differenziate (adulti e bambini, con viremia soppressa da altri regimi di cART, naive ed experienced).

I risultati saranno presentati ai prossimi congressi importanti e poi pubblicati, ma già da ora Merck annuncia che costituiranno la base per la richiesta di approvazione.

Islatravir Combined with Doravirine Continued to Maintain Viral Suppression for People Who Had Not Previously Received Treatment

KENILWORTH, N.J.--(BUSINESS WIRE)-- Merck (NYSE: MRK), known as MSD outside the United States and Canada, today announced Week 144 data from the Phase 2b dose-ranging study evaluating the antiretroviral activity, tolerability, and safety of islatravir in combination with doravirine compared to doravirine/lamivudine/tenofovir disoproxil fumarate (DOR/3TC/TDF) in antiretroviral treatment-naïve adults with HIV-1. Phase 2b data through 144 weeks demonstrated that islatravir combined with doravirine continued to maintain viral suppression, as measured by the number of study participants achieving HIV-1 RNA levels <50 copies/mL, similar to DOR/3TC/TDF. The data further characterized the tolerability and safety profile of islatravir in combination with doravirine. The Week 144 results are a follow-up to the Week 96 results and safety data presented at the virtual 2020 International Congress on Drug Therapy in HIV Infection (HIV Glasgow 2020) and the virtual International AIDS Society Conference on HIV Science (IAS 2021), respectively. The Week 144 data were consistent with the Week 96 results in the 0.75 mg dose group (the selected Phase 3 dose), and were shared as an oral presentation during the 18thEuropean AIDS Conference (EACS 2021) in London, U.K.

Islatravir is currently being evaluated across a variety of dosing regimens, for both the treatment of HIV-1 in combination with other antiretroviral agents and for the prevention of HIV-1 as a monotherapy. Week 144 Efficacy and Safety Results from Phase 2b Study of Investigational Islatravir with Doravirine

In this international, multicenter clinical trial (NCT03272347), treatment-naïve adult participants with HIV-1 were randomly assigned (1:1:1:1) to one of four once-daily oral treatment groups: islatravir 0.25 mg (n=29), 0.75 mg (n=30), or 2.25 mg (n=31) in combination with doravirine (100 mg) and 3TC (300 mg) compared to DOR/3TC/TDF (n=31; Part 1). After a minimum of 24 weeks of treatment, participants in the islatravir treatment groups with HIV-1 RNA <50 copies/mL were transitioned to a two-drug regimen consisting of doravirine and islatravir, without 3TC (Part 2). Participants in the islatravir treatment groups then transitioned to 0.75 mg islatravir (the selected Phase 3 dose) plus doravirine between Weeks 60 to 84, and they continued the combination therapy through Week 144 (Part 3). At Week 144, participants switched to the fixed-dose combination of the selected dose of islatravir and doravirine as open-label treatment until the end of the trial at Week 192 (Part 4).

At Week 144, at all dose levels, islatravir combined with doravirine maintained virologic suppression as measured by the proportion of study participants achieving HIV-1 RNA levels <50 copies/mL: 72.4% (n=21/29), 83.3% (n=25/30), and 61.3% (n=19/31) of participants maintained virologic suppression in the 0.25 mg, 0.75 mg, and 2.25 mg islatravir combined with doravirine groups, respectively. Overall, 72.2% (n=65/90) of the combined islatravir with doravirine groups had HIV-RNA levels <50 copies/mL, which was similar to 77.4% (n=24/31) of the DOR/3TC/TDF group. Through Week 144, seven participants met the criteria for protocol-defined virologic failure (PDVF) (confirmed HIV-1 RNA ≥50 copies/mL) and discontinued treatment, all of whom had HIV-1 RNA levels <80 copies/mL. No participants met the criteria for clinically significant confirmed viremia (HIV-1 RNA ≥200 copies/mL) or viral drug resistance analysis.

The proportion of participants experiencing at least one adverse event (AE) at Week 144 was similar between the islatravir combined with doravirine and DOR/3TC/TDF groups. At Week 144, 89.7% (n=26/29), 90.0% (n=27/30), and 77.4% (n=24/31) of participants experienced AEs in the 0.25 mg, 0.75 mg, and 2.25 mg islatravir combined with doravirine groups, respectively. Additionally, 85.6% (n=77/90) of the combined islatravir combined with doravirine groups experienced AEs, as compared to 87.1% (n=27/31) in the DOR/3TC/TDF group. The most common drug-related AEs for the combined islatravir combined with doravirine groups versus DOR/3TC/TDF were diarrhea (1.1% [n=1/90] vs. 12.9% [n=4/31), nausea (3.3% [n=3/90] vs. 9.7% [n=3/31]), headache (2.2% [n=2/90] vs. 3.2% [n=1/31]), and abnormal dreams [2.2% [n=2/90] vs. 0% [n=0/31]). No additional islatravir combined with doravirine participants reported drug-related AEs after Week 48. The rate of discontinuations due to drug-related AEs was 2.2% (n=2/90) for the combined islatravir with doravirine groups and 3.2% (n=1/31) for DOR/3TC/TDF, all occurring before Week 48. There were no deaths or serious-drug-related AEs in the islatravir combined with doravirine groups.
