La massima durata dello studio è di circa 6 mesi e secondo GSK già il prossimo maggio ci dovrebbero essere dei risultati sulla prima parte.

Che cosa sia questo GSK2838232 e in che cosa differisca da GSK353279, la cui sperimentazione sembra sia andata bene, ma che ciò nondimemo è stato bloccato, non so. Nel sito di GSK non c'è nulla al riguardo e quel che trovo in Google sono solo informazioni già viste.

Nuovi dati su sicurezza e tollerabilità dell'inibitore della maturazione di HIV GSK2838232

Jules Levin e Mark Mascolini riferiscono di dati presentati in questi giorni al 18th International Workshop on Clinical Pharmacology of Antiviral Therapy di Chicago sul nuovo inibitore della maturazione GSK2838232 e lo collegano al GSK2578999 presentato per la prima volta da GSK al CROI 2015, il cui poster può essere visto nel sito di Natap.

Due i lavori nuovi, svolti entrambi su gruppi di volontari sani:

2. Safety and Concentrations of GSK Maturation Inhibitor With or Without Ritonavir - questo il report di Mascolini:
  • A 100-mg ritonavir boost significantly raised exposure of GSK2838232, an HIV maturation inhibitor, in an analysis of four phase 1 dose-escalation and safety studies [1]. GSK investigators reported optimal plasma exposure of the investigational drug with repeated once-daily ritonavir-boosted dosing.

    By binding the HIV Gag protein, maturation inhibitors induce formation of immature viral particles that cannot infect other cells [2]. No HIV maturation inhibitors have been licensed or reached phase 3 trials. In 2016 GSK abandoned development of another maturation inhibitor, BMS-955176, because of poor results in a phase 2 trial [3].

    The analysis involved 124 healthy adults, 9 of them women, in four phase 1 trials outlined at ClinicalTrials.gov (NCT01802918, NCT02289482, NCT02289495, NCT02795754). Participants took single or repeat doses of GSK2838232 with or without a ritonavir boost. The trials tested both a solid formulation and an oral suspension. GSK2838232 doses ranged from 5 to 250 mg. One trial tested GSK2838232 for 11 days at doses up to 200 mg daily with 100 mg of ritonavir daily or 200 mg twice daily without ritonavir. Except for relative bioavailability assessments, the studies were placebo controlled.

    Four volunteers withdrew from NCT02795754 for adverse events judged unrelated to GSK2838232. Adverse events possibly related to GSK2838232 were headache and dizziness. Some participants had rash, probably reflecting wearing ECG electrodes for continuous cardiovascular monitoring. Sporadic nonsustained ventricular tachycardia affected 1 participant and was judged unrelated to GSK2838232. Hematologic and other lab values did not change significantly with GSK2838232.

    Unboosted repeat capsule doses of 200 mg twice daily with food resulted in trough concentrations above the target of 5 ng/mL. Once-daily single doses up to 100 mg and repeat doses up to 50 mg with an unoptimized suspension did not achieve target troughs.

    Ritonavir boosting raised single-dose GSK2838232 area under the concentration-time curve (AUC) 10-fold and maximum concentration (Cmax) 3-fold. Half-life rose from about 20 hours to about 34 hours with ritonavir. The capsule formulation achieved higher GSK2838232 exposure than the powder suspension. Food increased AUC and Cmax about 60% with the capsule formulation of GSK2838232.

    GSK investigators concluded that the pharmacokinetic profile of GSK2838232 indicates that the maturation inhibitor will maintain plasma concentrations above the protein-adjusted 90% inhibitory concentration for all HIV patients.

    A phase 2 study of 10-day monotherapy with cobicistat-boosted GSK2838232 in antiretroviral-naive adults with HIV is under way [4]. This is the first study of boosting with cobicistat rather than ritonavir.

[NdD: ho cambiato titolo al thread, così da poter parlare di tutta la classe di farmaci, perché con i passaggi di proprietà dei brevetti e con la sostituzione delle sigle si rischia di fare una grande confusione.]


GSK3640254: inibitore della maturazione di HIV sviluppato congiuntamente da ViiV Healthcare e GlaxoSmithKline completa la fase I e si appresta ad iniziare la fase II

Al 20th International Workshop on Clinical Pharmacology of HIV, Hepatitis & Other Antiviral Drugs che si è svolto in Olanda un paio di settimane fa ViiV e GSK hanno presentato i risultati del trial di fase I sulla sicurezza del loro candidato inibitore della maturazione.
Su Natap, J. Levin raccoglie le slides e M. Mascolini scrive una relazione, ripresa oggi pari pari su POZ da Benjamin Ryan.

In breve, in un primo studio randomizzato, con placebo e dosaggi crescenti del farmaco (assunto oralmente in capsule) su circa 80 persone, l'inibitore della maturazione si è dimostrato sicuro e ben tollerato e non ci sono stati eventi avversi gravi.
In un secondo studio, che si è svolto su 14 persone, si sono indagate farmacocinetica e farmacodinamica di due diverse versioni del farmaco.
Mettendo insieme i risultati delle due sperimentazioni, gli eventi avversi sono stati 9, 4 dei quali abbastanza seri da spingere i partecipanti ad abbandonare lo studio.
Gli eventi avversi più comuni: mal di testa, dermatite da contatto (dovuta agli elettrodi dell'EGC), diarrea, vertigini, ematomi, fatigue e mal di schiena.
L'assorbimento del farmaco è lento, richiede circa 4 ore per arrivare ai livelli massimi. Poiché ha un'emivita di circa 24 ore, si presta all'assunzione una volta al giorno.

Sta già iniziando la seconda fase del trial su 34 persone HIV+ naive - randomizzato, in doppio cieco e con placebo:

Background: Drug resistance and toxicities with HIV-1 regimens can result in treatment failure, so antiretroviral agents with new mechanisms of action are needed. GSK3640254 (GSK'254) is a novel, next-generation HIV-1 maturation inhibitor with pharmacokinetics (PK) to support unboosted, once-daily therapy and a desirable drug-drug interaction profile.

Methods: This phase IIa double-blind (sponsor unblinded), randomized, placebo-controlled, adaptive study evaluated the antiviral effect, safety, tolerability, and PK of once-daily GSK'254 administered with a moderate-fat meal in treatment-naive adults infected with HIV-1. In part 1, participants received GSK'254 10 or 200 mg for 10 days. In part 2, participants received GSK'254 40, 80, or 140 mg for 7 days, modified from 10 days by a protocol amendment to decrease potential for treatment-emergent resistance- associated mutations (RAMs) observed after 7 days in part 1. Primary endpoint was maximum change in plasma HIV-1 RNA during parts 1 and 2. Secondary endpoints were safety, tolerability, and PK parameters.

Results: Of 34 participants who received GSK'254 (n=6/dose) or placebo (n=4), 94% were men; mean age was 31.8 years. Mean changes in plasma HIV-1 RNA ranged from -2.0 to 0.2 log10; the largest decreases of 2.0- and 1.5 log10 occurred in the 200- and 140-mg groups, respectively (Table). PK results were generally dose proportional and consistent with those observed in non-HIV-infected individuals. 4/6 participants in the 200-mg group (part 1) developed RAMs on Day 11; 1 of these 4 developed phenotypic resistance. No RAMs were observed at any dose in part 2. No deaths were reported. Two non-drug-related serious adverse events (AEs) of congestive cardiomyopathy and anal abscess occurred. No AEs led to discontinuation. AEs were reported by 22 (65%) participants, with the most common being headache (n=4). Drug-related gastrointestinal AEs, including diarrhea (n=3), abdominal pain (n=2), and vomiting (n=2), occurred in 6 (18%) participants. Conclusion: This short-term monotherapy study established a dose-antiviral response relationship. Regardless of dosing duration, GSK'254 140- and 200-mg doses demonstrated greatest declines in plasma HIV-1 RNA. No safety or tolerability concerns were noted. These results support the ongoing phase IIb study (ClinicalTrials.gov identifier: NCT04493216).

As with all drugs in new classes, maturation inhibitors would be active against resistance to other drug classes. GSK254 shows good antiviral activity.

The early cases of drug resistance though show a lower genetic barrier to drug resistance than PIs and NSTRIs and this will make it essential to be used in combination with other active drugs. (*)

Il trial di fase IIb in corso - studio DOMINO:

A Dose-Range Finding Clinical Trial Study in Human Immunodeficiency Virus (HIV-1) Infected Treatment-Naive Adults (DOMINO)
GSK, cioè ViiV Healthcare per quanto riguarda il mercato dell'HIV, ha deciso di non proseguire nelle sperimentazioni dell'inibitore della maturazione GSK3640254, che pure - come abbiamo visto nel 2021 e inizio 22 - ha dimostrato attività antivirale, sicurezza e tollerabilità, perché questo candidato farmaco non si differenzia abbastanza dagli altri antiretrovirali orali da assumere quotidianamente che già sono in commercio.
Inoltre, se assunto poco dopo un pasto, l'efficacia di GSK3640254 diminuisce - il che lo rende evidentemente poco pratico.

Rimane in studio un altro inibitore della maturazione, VH3739937, attualmente in fase I di sperimentazione.
