Un comunicato stampa di Merck annuncia il completamento con successo della sperimentazione di fase II della doravirina in pazienti naive e l'inzio del trial di fase III entro la fine di quest'anno.
I dati a 48 settimane sono stati presentati ieri da Josef Gatell al 12th International Congress on HIV Drug Therapy, in corso a Glasgow e confermano quelli ad interim di cui abbiamo già parlato (abstract #O434).

  • The primary safety analysis from the expansion phase of the study compared the incidence of central nervous system (CNS) adverse events (AEs) by Week 8 in patients who received doravirine 100 mg plus TDF/FTC (n=108) versus patients who received efavirenz with TDF/FTC (n=108). The results showed a significantly lower incidence of one or more of reported CNS AEs (all causality) among the doravirine-treated group compared to the efavirenz-treated group (22.2 % vs. 43.5 %, respectively; p<0.001). The most common (occurring in more than 5 percent of patients) CNS AEs in the doravirine-and efavirenz-treated groups, respectively, were dizziness (9.3 % vs. 27.8 %), insomnia (6.5 % vs. 2.8 %), abnormal dreams (5.6 % vs. 16.7 %) and nightmares (5.6 % vs. 8.3 %).

    Interim results for this ongoing Phase 2b study, including the primary efficacy analysis for dose selection based on 24-week data from the dose-ranging cohort of the study, were previously presented at the 21st Conference on Retroviruses and Opportunistic Infections (CROI) in March 2014.

    “This program underscores Merck’s ongoing commitment to the research and development of new therapeutic options for patients with HIV,” said Dr. Hedy Teppler, executive director, Infectious Diseases, Merck Research Laboratories. “We are encouraged by the antiviral activity and the overall tolerability profile of doravirine and look forward to initiating Phase 3 studies.”

    Additional follow-up data through 48 weeks of treatment showed a 76 percent (n=126/166) overall virologic response rate (HIV RNA <40 c/ml) for all doravirine doses (25, 50, 100 and 200 mg) that is comparable to 71 percent (n=30/42) reported for patients administered efavirenz (600 mg). In addition, all treatment groups showed increased CD4 cell counts relative to baseline, consistent with the 24-week findings.

    After 48 weeks of treatment, patients in the dose ranging part of the study who received doravirine demonstrated a lower overall incidence of drug-related adverse events (36.7%; n=166) than those who received efavirenz (57.1%; n=42). The most commonly reported drug-related clinical adverse events in the doravirine and efavirenz groups respectively were abnormal dreams (10.2% vs. 9.5%); nausea (7.8% vs. 2.4%); fatigue (7.2% vs. 4.8%); diarrhea (4.8% vs. 9.5%) and dizziness (3.0% vs. 23.8%). Doravirine-treated patients also had a lower incidence of laboratory abnormalities in routine clinical tests including increased total cholesterol (6.8% for doravirine vs. 31.6% for efavirenz) and LDL cholesterol (6.3% for doravirine vs. 18.4% for efavirenz).


    Merck plans to initiate the first Phase 3 clinical trial of doravirine by the end of 2014, NCT02275780. The study will enroll treatment-naïve patients and compare the efficacy, safety and tolerability of doravirine and ritonavir-boosted darunavir, both in combination with other anti-retroviral therapy.

Una domanda,se si sviluppa resistenza all'efavirenz,la terapia non potrebbe essere sostituita con la migliore Doravirina?Be' anche se non dovessi sviluppare resistenze all'efavirenz chiederei alla mia infettivologa la sostituzione con la Doravirina,visto che,in base a questi trial, l'incidenza di effetti collaterali è(dovrebbe essere) più bassa. Ma chissà quando arriverà in Italia! :roll:

IAS 2015: Efficacy and Safety of Doravirine 100mg QD vs Efavirenz 600mg QD with TDF/FTC in ART-Naive HIV-Infected Patients: Week 24 Results (slides su Natap a breve).

In persone con HIV naive, dopo 24 settimane di assunzione, 100 mg di doravirina + [TDF/FTC] (Truvada) al giorno hanno confermato:

  • - un'attività antiretrovirale e un effetto immunologico simile a quello di [efavirenz + TDF/FTC] (Sustiva);
    - stessa sicurezza e tollerabilità;
    - significativamente meno e meno gravi effetti sul sistema nervoso centrale.

Di recente sono casualmente venuta a sapere che esiste un retroforum in cui persone che mi detestano e amano tirarmi fango addosso usano il nome del candidato NNRTI di Merck come "prova" dei miei oscuri rapporti con Big Pharma.

I risultati presentati sono quelli della sperimentazione di confronto con Efavirenz a 48 settimane, cioè a metà della seconda parte di questo lungo lungo trial di fase II, la cui impostazione è sintetizzata qui:

La slide che segue ci mostra una panoramica dello stato dei partecipanti alla 48° settimana, da cui si evince che i pazienti del gruppo della doravirina hanno abbandonato in minor numero lo studio, hanno avuto meno effetti avversi, sono stati persi di meno al follow-up, ma per qualche ragione sono stati anche in minor numero conformi alle regole di assunzione del farmaco.

Sono felice di comunicarvi che la ImmagineDoravirinaImmagine continua ad andare benissimo e ha centrato l'obiettivo primario dell'efficacia nel trial di fase III di confronto con darunavir/ritonavir dopo 48 settimane di trattamento:
  • - non inferiorità di DOR rispetto a DRV+r dal punto di vista della soppressione della viremia (anche viremie alte)
    - basso tasso di resistenze: 1 solo partecipante su 383 ha sviluppato mutazioni che l'hanno reso resistente al farmaco
    - superiore profilo lipidico di DOR rispetto a DRV+r
    - ottimo profilo di tollerabilità e basso tasso di abbandoni dovuti ad eventi avversi

Merck’s Doravirine, an Investigational Non-Nucleoside Reverse Transcriptase Inhibitor (NNRTI) for the Treatment of HIV-1 Infection, Met Primary Efficacy Endpoint in Pivotal Phase 3 Trial

Data Presented at CROI Showed Doravirine Was Non-inferior to Ritonavir-boosted Darunavir in Treatment-naïve Adults after 48 weeks of Treatment
  • [...] The study met its primary efficacy endpoint of the proportion of participants achieving levels of HIV-1RNA less than 50 copies/mL after 48 weeks of treatment, demonstrating the non-inferiority of once-daily doravirine (DOR) to once-daily ritonavir-boosted darunavir (DRV+r), each administered with tenofovir disoproxil fumarate/emtricitabine (TDF/FTC) or abacavir/lamivudine (ABC/3TC), in previously untreated (treatment-naïve) adults with HIV-1 infection. In addition, a secondary endpoint showed that the DOR-treated group had statistically significant lower levels of fasting low density lipoprotein cholesterol (LDL-C), versus the DRV+r group. Findings from the ongoing “DRIVE-FORWARD” Phase 3 trial following 48 weeks of treatment were presented as a late-breaking abstract at the annual Conference on Retroviruses and Opportunistic Infections (CROI) being held in Seattle from Feb. 13-16, 2017.

    “Improved understanding of the biology of HIV and growing clinical evidence from current therapies are advancing the management of HIV infection,” said Dr. Kathleen Squires, professor and director of infectious diseases, Thomas Jefferson University, Philadelphia. “The results of this study provide solid evidence of the efficacy and safety profile of doravirine as a potential treatment option for treatment-naïve HIV-1 patients.”

    Data from DRIVE- FORWARD

    In the trial, after 48 weeks of treatment, the proportion of participants achieving levels of HIV-1 RNA less than 50 copies/mL following once-daily DOR (100 mg) or once-daily DRV+r (800 mg and 100 mg respectively), each in combination with either TDF/FTC or ABC/3TC, was 83.8 percent (321/383) and 79.9 percent (306/383), respectively; with a treatment difference (95 % confidence interval) of 3.9 [-1.6, 9.4]. Increases in mean CD4+ T-cell counts from baseline were similar for the DOR and DRV+r treatment groups: 193 and 186 cells/mm3, respectively.

    In addition, comparable efficacy was observed for participants with baseline levels of HIV-1 RNA greater than 100,000 copies/mL: 81.0 percent (64/79) for DOR and 76.4 percent (55/72) for DRV+r; with a treatment difference (95% confidence interval) of 3.0 [-11.2, 17.1]. One out of 383 participants developed phenotypic and genotypic resistance in the DOR arm (the patient came off study at Week 24 because of non-adherence). None of the 383 participants receiving DRV+r developed phenotypic and genotypic resistance.

    The rates of reported adverse drug reactions were 31 percent (117/383) for DOR and 32 percent (123/383) for DRV+r. Discontinuations due to adverse events for the DOR and DRV+r treatment groups were 2 percent (6/383) and 3 percent (12/383), respectively. The most common adverse events for DOR and DRV+r (occurring in greater than or equal to 10 percent of participants) were: diarrhea (14% vs. 22%); headache (14% vs. 11%); nausea (11% vs. 12%) and nasopharyngitis (8% vs. 10 %), respectively.

    Analysis of fasting serum blood lipids for the DOR and DRV+r treated groups showed a statistically significant difference (p<0.0001) in mean changes from baseline in the levels of LDL-C (-4.5 mg/dL vs. +9.9 mg/dL) and non-high density lipoprotein cholesterol (non-HDL-C) (-5.3 mg/dL vs. +13.8 mg/dL), respectively. Mean changes from baseline in total cholesterol, high density lipoprotein cholesterol (HDL-C) and triglycerides for the DOR- treated group and the DRV+r- treated group were -1.4 mg/dL, +3.9 mg/dL, and -3.1 mg/dL vs. +17.9 mg/dL, +4.2 mg/dL, and +22 mg/dL, respectively.

    “Merck has been at the forefront of research into HIV for three decades,” said Dr. George Hanna, associate vice president, clinical research, Merck Research Laboratories. “We are encouraged by these results, which provide additional insights into the efficacy and safety of doravirine.”

    About DRIVE-FORWARD

    DRIVE-FORWARD is a multicenter, double-blind, randomized non-inferiority trial in which 769 treatment-naïve adults with HIV-1 infection received either 100 mg doravirine or 800 mg darunavir plus 100 mg ritonavir, both administered orally once-daily in combination with either TDF/FTC or ABC/3TC. The primary endpoint of the clinical trial was the proportion of participants with HIV-1 RNA copies of less than 50 copies/mL at Week 48. There were a number of secondary endpoints, including an evaluation of the effects of DOR and DRV+r on fasting serum lipids, change from baseline in CD4+ T-cell count, and evaluation of safety and tolerability.
