Credo di no. Penso che l'avrebbero scritto, ma non lo vedo da nessuna parte.
E un'altra cosa di cui non hanno parlato è la Graft, che in Timothy c'è stata, per quanto lieve, e che potrebbe aver avuto un ruolo importante.

Ho un dubbio. In caso di trapianto di cellule staminali CCR5 difettive che, oltre alle leucemie, eradichi anche l'hiv come avvenuto in Timothy e forse in questo paziente tedesco, non rimarrebbe il reservoir nel cervello costituito dagli astrociti? Gli astrociti non derivano da staminali emopoietiche, no?

Non è detto che ci sia un reservoir lì e che se c'è sia di dimensioni importanti. Tieni poi conto che se c'è del virus che esce dal SNC, non trova CD4 CCR5+ da infettare.
Comunque, la total body irradiation arriva anche al cervello (anche se un po' lo si deve proteggere). Infatti mi ha colpito che al paziente di Düsseldorf non l'abbiano fatta.
Dovrebbero pubblicare presto un articolo e lì troveremo senz'altro molti nuovi dettagli.

Giustamente anche se dovesse esserci qualche residuo di virus, vedendosi chiusa la porta del CCR5+ , non ci sarebbe alcun modo per replicarsi.
Conosco la prassi ahimè, un membro della mia famiglia sta facendo radioterapia per una metastasi cerebrale con il casco stereotassico e non è una passeggiata.

Concerning HIV, all collected samples were negative for proviral DNA by conventional and digital droplet PCR* in two different labs, namely PBMCs (06/2014, 01/2015* and 02/2015), rectal biopsy (04/2015) and bone marrow (08/2015*). Western blots from 06/2014 and 02/2015 showed incomplete patterns with fading bands.
Dora perdonami, forse ho capito male: è stato misurato anche il DNA provirale nei tessuti, ma com'è possibile che non ci sia traccia di provirus se non si è stimolata in alcun modo la trascrizione del provirus latente? Nel paziente di Berlino erano state effettuate misurazioni del reservoir? Capisco che la mutazione di fatto impedisca al virus che si riattiva di legarsi alle cellule, ma "come" e soprattutto "in quanto tempo" avviene questa riattivazione?

L'emivita media dei reservoir è di 44 mesi, quindi secondo me prima di una decina di anni non possono giungere a nessuna conclusione, salvo autopsie

Beh una conclusione definitiva e assoluta no, però nel caso di Timothy si può essere ragionevolmente sicuri che di virus capace di replicazione non ce ne sia più. 9 anni senza rebound e già da 4 anni lo si considera eradicato.
Per questo nuovo, vedremo che troveranno nei linfonodi, che ancora non sono stati analizzati.


Rospi, se di provirus non ne hanno trovato, potrebbe anche darsi che non ce ne sia più, distrutto dal condizionamento pre-trapianto. In quel caso sarebbe guarito. :-D
E sì, il Berlin Patient è stato rivoltato come un calzino per cercare DNA virale. Quel che si è trovato (e solo alcuni laboratori, mentre altri niente) sono state debolissime tracce di proteine virali. Nulla che potesse dar inizio a processi di replicazione. Infatti nulla ha ricominciato a replicarsi.

All'HIV Drug Therapy di Glasgow è stato presentato il caso del "DUSSELDORF PATIENT", che presenta grandi somiglianze con quello di Timothy Brown, solo che per ora sta ancora prendendo la ART.

Ne ha raccontato Mark Mascolini su NATAP:

HIV DNA Vanishes ["Cure"?] After CCR5-delta32 Stem Cell Transplant, But ART Continues

Another HIV patient with acute myeloid leukemia (AML) is responding to a CCR5-delta32 stem cell transplant with disappearance of proviral DNA from all samples scrutinized [1]. A delta32 stem cell transplant cured the Berlin patient of HIV infection [2], but the new "Dusseldorf patient" has yet to stop antiretroviral therapy (ART) to confirm a cure.

The Berlin patient apparently remains the only person cured of HIV infection after allogeneic stem cell transplantation (also for AML) from a donor homozygous for the delta32 deletion [2]. This well-known man has remained free from HIV for nearly a decade without ART. At the Glasgow meeting, researchers from the University of Cologne and other centers reported the case of a similar patient who has yet to suspend ART.

Like the Berlin patient, the Dusseldorf patient had AML, diagnosed in January 2011 only months after his HIV diagnosis. This 41-year-old man began ART with darunavir/ritonavir plus tenofovir/emtricitabine and in January 2011 had a plasma viral load of 44 copies and a CD4 count for 474. In March 2011 his clinicians switched from darunavir to raltegravir to avoid potential interactions during chemotherapy.

The man achieved complete remission of AML after two ICE induction courses and three consolidation courses. When AML relapsed in September 2012, the patient responded to A-HAM plus high-dose cytarabine. During a second complete remission, he received a transplant of unmodified peripheral blood stem cells from a donor homozygous for CCR5-delta32 in February 2013, after conditioning with fludarabine and treosulfan. Before the transplant HIV DNA measured 1.45 log10 copies/million peripheral blood mononuclear cells (PBMCs) and detectable western blot bands confirmed HIV infection.

A second AML relapse occurred in June 2013, and this time the patient responded to eight courses of 5-azacytidine and four donor lymphocyte transfusions. Since June 2014 the patient has taken dolutegravir plus abacavir/lamivudine and viral load remained undetectable in plasma.

All collected samples now test negative for proviral DNA by conventional PCR and digital droplet PCR performed at two different labs. Assayed samples include PBMCs (2014-2016), rectal biopsy (April 2015), and bone marrow (August 2015). The researchers saw incomplete patterns and fading bands on western blots done in 2014, 2015, and 2016. Viral outgrowth assays in PBMCs are negative. HIV LTR DNA cannot be detected in PBMCs by digital droplet PCR or quantitative PCR, but the assays see weakly positive signals in T-cell subsets.

The research team plans further assays including proviral DNA and quantitative viral outgrowth assays in lymph nodes, plus cellular immune response assays, before considering stopping ART. But they believe tests so far--in the 4 years since the stem cell transplant--"suggest that HIV may have been eradicated and that he may be the second individual cured from HIV by allogeneic CCR5-delta32" stem cell transplant.

  • The Berlin patient is presumed to be the only person cured from HIV infection by hematopoietic stem cell transplantation (HSCT) from a homozygous CCR5-d32 donor. Attempts to reproduce cure by HSCT have failed because of either viral rebound or death due to the underlying malignancy. We here report a patient alive, well and negative for proviral DNA 900 days after HSCT. A 41-year-old HIV-infected male patient was diagnosed acute myeloid leukaemia (AML, inv16, CBF-MYH11) in January 2011. Since the diagnosis of HIV infection in October 2010, he had been treated with TDF/FTC/DRV (January 2011 VL 44 copies/mL; CD4 474 cells/mL). To avoid interactions with chemotherapy DRV was switched to RAL in March 2011. He achieved CR of the AML after one induction course (ICE)and received a second induction and three consolidation courses according to AML-SG 07/04. In September 2012, AML relapsed and he was treated with A-HAM and a second cycle high-dose cytarabine.
    While in second CR, he received unmodified peripheral blood stem cells from a female 10/10 CCR5-d32 DKMS donor after conditioning with fludarabine/treosulfan in February 2013. Before transplant, HIV resistance analysis was performed and viral tropism was determined.
    There were no significant resistance mutations, and the coreceptor usage was predicted as R5-tropic (Sanger sequencing: FPR 44.5%; NGS: 0.14% X4 at 3.5% FPR; geno2pheno). The proviral DNA load was 1.45 log10 copies/106 PBMCs, and in the western blot, all anticipated bands could be detected. During transplant and until today, the patient remained on ART (since June 2014 ABC/3TC/DTG), and the viral load remained undetectable in plasma and liquor. He had a second relapse of AML in June 2013 but re-entered molecular remission after a total of eight courses of 5-azacytidine and four donor lymphocyte infusions.
    Concerning HIV, all collected samples were negative for proviral DNA by conventional and digital droplet PCR in two different labs, namely PBMCs (20142016), rectal biopsy (April 2015) and bone marrow (August 2015).
    Western blots from 2014, 2015 and 2016 showed incomplete patterns with fading bands.
    Viral outgrowth assays are in progress. Like in the Berlin patient, all tests from the Duesseldorf patient so far suggest that HIV may have been eradicated and that he may be the second individual cured from HIV by allogeneic CCR5-d32 HSCT. Further investigations will be performed before considering the discontinuation of ART.

In un post pubblicato ieri per HIV i-Base, Simon Collins segnala un possibile nuovo caso di eradicazione in UK, cui ha accennato in due slides Ian Gabriel, Chelsea and Westminster Hospital, London, durante una sessione sui trapianti di midollo alla BHIVA Autumn Conference 2018.
Le slides di Gabriel non sono online, quindi per saperne di più bisogna attendere i prossimi congressi - direi il CROI a inizio marzo.
Questo è tutto quel che ci racconta Collins:
  • The case involves an HIV positive man who has undergone successful allogeneic stem cell transplantation from an unrelated donor homozygous for CCR5 delta-32 deletion.
    ART was stopped 15 months post transplant following ethics approval and for the subsequent 12 months HIV has been undetectable by all methods including proviral DNA outgrowth and integrated and total HIV DNA in CD4 cells.
